Meredith is upset over the hurtful things her mother said to her while she was lucid. Derek finds her completely underwater in her tub, and is afraid she was trying to drown herself. She tells him she's fine. The Chief has dyed his hair now that he's single again. Burke wonders why Cristina hasn't told her friends that they are engaged. Callie worries that George isn't respecting her position as his boss now that they're married. While the residents are doing a triage drill, they are called to the scene of a mass casualty incident - a ferry that got in an accident and caught fire. George helps a woman who won't go into her much-needed surgery until someone finds her 7-year-old son. Alex rescues a pregnant woman who was crushed. Izzie tries to help a man who is trapped under a car on the ferry. A lost little girl attaches herself to Meredith. While helping a man with a leg injury, Meredith is knocked into the ocean. No one sees what happened and the little girl silently walks away.
